Case Report 1: Module 4. Population Health Interventions
4A) Background:
In addition to improving surveillance, the stakeholder advisory board made recommendations, based on the population health assessment, to improve the quality of mental health care provided by City C’s health
system to LGBTQI populations. Consistent with the priorities identified during the population health assessment, the stakeholder advisory board
recommended focusing on interventions that are cost-effective, and/or directly connected to reducing suicide attempts.
4B) Reflection Question:
What should City C’s health system consider when selecting, implementing, and evaluating population health interventions to improve mental health care for LGBTQI populations?
4C) Considerations:
Selecting
· Interventions selected should have demonstrated cost-effectiveness OR an impact on reducing outcomes connected to suicide attempts.
· Interventions selected should have demonstrated effectiveness in the population of interest: LGBTQI populations.
· Interventions selected should capitalize on the assets of the health system and the surrounding community.
· The stakeholder advisory board provided input on the interventions selected.
Implementing
· A plan, do, study, act framework was created to begin the implementation process. The stakeholder advisory board provided input on the framework.
· The healthcare system prioritized making environmental changes throughout their hospitals and clinics based on guidance from the Gay and Lesbian Medical Association on creating a welcoming clinic space for LGBTQI patients (Gay and Lesbian Medical Association, n.d.).
· With guidance from the stakeholder advisory board, the healthcare system identified relevant trainings in LGBTQI-affirming care.
· For example, the health system’s healthcare employees were educated on appropriate referral sources and learning how to approach conversations about LGBTQI specific healthcare needs.
· The healthcare system prioritized hiring clinicians with experience in treating LGBTQI individuals and providing LGBTQI affirming care.
Evaluating
· The plan, do, study, act framework helped guide the evaluation process.
· Quality improvement efforts focused on examining EHR mental health data and satisfaction data from patients who identify as LGBTQI.
· The year after changes were implemented, the healthcare system conducted focus groups with employees and patients to gather in depth qualitative data about the extent to which the changes improved care for LGBTQI populations.
· Evaluation results were shared regularly with the stakeholder advisory group, employees, patients, and the larger community.
· The results, and feedback received upon sharing the results were used to select and implement additional interventions to further improve care for LGBTQI populations.
