Avoiding Hospital Readmission

Clinical Question

In the United States we face many challenges that impacted the healthcare system. Our

health professionals must collaborate to seek solutions in achieving goals that are best for patient

care. The purpose of this research paper to introduce to you a major change that Medicare and

Medicaid have to do with readmissions. One study found among hospitals that between five

percent and fifty nine percent of readmissions can be avoided, but based on the evidence

reviewed that was shown in this article about fifthteen percent up to twenty percent would be

considered reasonable although previous authors have advised against producing a benchmark

figure for the percentage of readmissions that can be avoided ( Ellen Nolte, Martin Roland,

Susan Guthrie, Laura Brereton, Rand Health Q, 2012). I will be comparing two articles that I

have chosen and will determine if the data found will support or answer the PICOT question.

How many people in the elderly population use Medicare and Medicaid to reduce the risk of

hospital 30 days readmission rates compared to those that do not have Medicare and Medicaid?

Levels of Evidence

In my readings the question implemented in my paper is a PICOT question that shows the

main focal point on the diagnosis and the articles provided by Chamberlain library study articles

that are available for the students. My credible sources are considered to be the primary source of

definition studies since they are the original studies (Houser, J., 2018). A cross-sectional study is

the best medical research to use because it will answer questions that consist of observational

study from analyzing data that is very interesting.This medical research focused on a subject

matter that is studied at a single point in time (Houser, J., 2018). The main articles that I input

based on data that are gathered by the readmission prevention program.

Search Strategy

The Chamberlain College of Nursing online library has a lot to offer for their nursing

students when looking for research articles. When I typed my topic there were several different

topics that I used to find the articles that best fit my RUA. For example, when I typed

“preventing hospital readmissions” it helped me find the second article for my research paper

that lead to searching “avoiding hospital readmissions”, “reducing hospital readmissions”. I

would encourage my cohort to utilize The Chamberlain College of Nursing online library

because it was more accommodating that lead to my main credible source to find my research

articles. Google Scholar didn’t work for me because I couldn’t find credible sources and the

website does not have as many filters to help me to narrow down the best articles. The process

that I used to narrow the information down was time-consuming because I had to rephrase what I

was looking for, filtering the years and any articles that are not among peer-reviewed.

Unfortunately, filtering this way did not give me the result that I was looking for that would be

helpful to incorporate into my paper due to data that occurred in 2009.

Article One

The first article that was effective in my research focused on Centers for Medicare &

Medicaid Services and commercial healthcare payers that punished hospitals when patients are

readmitted. This article is based on a quantitative study to create discharge plans that would go

over and beyond to coordinate with providers to help patients to follow their plan. It also

provides some background that it is important to develop a relationships with providers by

meeting with them face to face, discussing ways to improve transitions and solve problems ("To

prevent readmissions, think outside the hospital walls", Dec. 2015). They also keep records of

the readmission rates, share it with the providers then inform the results to the group to work on

a solution.

Article Two

The second article that I chose aims at healthcare experts and providers that worked hard

to come up with a solution to reduce readmissions. It is important to collaborate with the

financial and clinical leaders to help us comprehend to stay within the guidelines before the

hospital be penalized by the Centers for Medicare & Medicaid Services regulations. It is

important to get advice and to be knowledgeable about what the impact will be in the future.

Next, they have to be proactive to come up with strategies for later time based on the potential

changes due to readmission. The strategy that they used talked about the human and societal

benefits of reducing avoidable readmissions, the financial fallout of the regulations that is

unknown, providers, reducing readmissions that help reductions in revenue until providers start

using the Financial Impact Diagnostic Tool to help with impact (Birk S, 2012).

Conclusion

The articles provided in my research paper are useful credible sources that I implemented

with great information about collaboration with healthcare experts, providers, clinical leaders, financial

and Centers for Medicare & Medicaid Services to identify preventing readmissions. Education is very

important working in the healthcare field to be able to use the aspect of quality care and strategies so, we

can provide the best patient to reduce hospital readmission. Both articles that were presented in my research

more direct support that help me prove the purpose of my research question. There might be some

alterations doing this research process but it was worth the change. However, I am looking forward to

exploring this topic of my research question and collaborate with the other member’s articles to provide the

best information.
