List the financial, material, personnel, and other resources available to be used as inputs to address the problem. This should include the staff, volunteers, partners, time, research material, equipment, etc.
Define the public health problem.
These are the ultimate impacts in public health. They make take years to accomplish. They are the result of decisions, policies, environmental and civic changes arising from public health actions.
These outcomes often take more time. They may include actions, decision-making, policies, and social actions.
These are short-term results. Typically these are items that can be accomplished and measured within a few weeks to months. They include things we might learn such as gains in knowledge, awareness, changes in attitudes and behaviors.
Provide a list of the things you intend to do. This will include classes, workshops, assessments, recruiting, mobilizing, media outreach, etc.
This is who you intend to reach in the program. It may be patients, clients, agencies, customers, and other stakeholders.
