Population: Congestive heart failure patient age group 19-65 years of age.

Intervention: educational approach.

Comparison: Educational methods utilized for inpatient and outpatient Congestive heart failure teaching

Outcome: identify educational approaches that helps in the prevention of readmission

Time: Prevention of readmission in 30 days.

In patients 65 years of age and older with congestive heart failure, how does Multidisciplinary education approach lower rate of readmission to the hospital within 30 days of discharge with being compliant compare to being noncompliant?
