Offense(s) for which child was placed under supervision: (List case # and charge(s))

Details of Offense: List the specifics of the complaint that the child was adjudicated for and placed under supervision. Discuss any relative information which would shed light on the circumstances surrounding the offense. (i.e. why, what, when, how).

 

Legal History: Discuss any previous contacts with the Juvenile Court in this county as well as any contacts with courts in other counties and/or states.

 

Family Data

Father: Is child's father absent or present in the home? Is there a step-father or other male care-taker? Describe his role in the home-relationship with spouse, type and consistency of discipline, employment (i.e. what type of job, income hours worked, etc.), Are there any outstanding problems that need to be addressed (medical, emotional, drug/alcohol)

Mother: Describe same information as the father above

Other Significant Adults: age, sex, relationship to the child, do they live in the home?

Siblings: Does child have any siblings, do they live in the home? sex/gender, are any of them positive resources for this child, are there any outstanding problems that need to be addressed?

Home Life: Describe housing, neighborhood. Have there been any traumatic experiences (i.e. deaths, illnesses, divorces, etc.) Describe the basic feelings of family members towards each other (caring, hostile, indifferent). Has the family been referred to or involved in counseling? Are there any issues that need to be addressed?

Child’s Perception of Current Situation: How does the child perceive his/her present situation and how has the court's involvment impacted the lives of the child and his/her family?

 

Child’s Personal History and Adjustment

Physical Health

  1. Medical History: Discuss child's general physical description, any serious illnesses or handicaps
  2. Drug/Alcohol Involvement: Has child ever used drugs or alcohol, does child have a history of substance abuse, have drugs or alcohol been involved in any of the child's previous offenses? Are there any outstanding issues here that need to be addressed?

School – Academic & Vocational Aptitude: What school does child attend, what grade, does child need special education services or have an IEP? Discuss school attendance, achievement, behavior and extra-curricular activities. Does child have any career goals, work experience or vocational interests?

Leisure Time: What does child do for fun? Does he/she have any special interests or talents?

Previous Treatment Efforts: Has child ever had psychological evaluation or been referred to counseling? has child ever been placed in a residential program? What other agencies are now or have become involved with child?

 

Supervision Plan

What is the probation officer's plan for treating and rehabilitating child? Recommendations should be thought out and should include justification for each recommendation based on outstanding problems that were identified above. List goals for the child and explain how you as the probation officer intend to help the child and family reach those goals. Goals should be realistic and attainable.
