WORKSHEET 10.1A: Summary Questionnaire

Use the filled-out Worksheet 10.1B in the book as an example to follow as you complete this Questionnaire.

1. What is the identity of your organization, and what is its mission?

The indentity of our organization is called Kids In Distress (KID), which focuses on helping kids in Distress. The organization seeks to prevent child abuse, preserve families and treat children who have been abused or neglected. The organization's mission is to "prevent child abuse, preserve the family and treat children who have been abused and neglected" (Kids In Distress, n.d.).

2. What is the proposed project (title, purpose, target population)?

The project aims to raise money to help children who have been abused and neglected live everyday life. The target population is young children from unstable families who have been abused or neglected. In addition, the project seeks to prevent a child at high risk for abuse from being used by providing a safe environment for them (Kids In Distress, n.d.).

3. Why is the proposed project important?

The proposed project seeks to bring a positive change to the community. Many kids are in Distress because of being brought up in a toxic environment, where they are likely to be abused or neglected. For example, some kids are under the care of parents who are drug addicts. Kids from such family settings are vulnerable to being abused or neglected. Therefore, the organization seeks to work with these kids to help them live everyday lives, attend school, have food, health care, and proper housing.

4. What will be accomplished by this program or project during the time period of the grant?

The project seeks to help more than 500 kids who have been abused or neglected or those at the risk of being abused or neglected by providing emergency shelters to abused, neglected kids, or abandoned infants. The project also seeks to provide counseling services and health care services to abused and neglected kids. In addition, the project will partner with educational institutions and government institutions to ensure that abandoned kids enroll in school.

5. Why should your organization do the project (credibility statement)?

KID organization has been in existence since 1976. For more than 45 years, the organization has partnered with many donors and has provided help to thousands of neglected and abused kids. The organization has positively impacted many families by helping children in need and is one of the most reputable nonprofit organizations in South Florida. The organization has no single case of embezzlement of funds made to help kids in Distress.

6. How much will the project cost during the grant time period? How much is being requested from this funder?

The project seeks to provide a myriad of services to more than 500 kids in Distress within Florida, including health care, mental health services, food distribution, and emergency shelter. Therefore, a significant amount totaling $3,000,000 is needed to help fund the project during the grant period. Since the organization has currently exhausted its finances and relies on donors for funding, the exact amount is requested from the funder. The funds requested effectively reflect the organization's need to fund the project (O'Neal-McElrath, 2013).
