Introduction Health promotion is aimed at ensuring that there is maintenance of healthy individuals by offering them the required education tools and resources. Health promotion program is developed to help people to choose and adopt healthy behaviors that help in the reduction of the risk of developing chronic illnesses. Tobacco use is among the focus of Healthy People 2020 (Centers for Disease Control, 2019). The adolescent suffers majorly from this health issue. This population are at risk of smoking experimentation. 1Based on the reports from the CDC, about 12 out of 100 middle school students and 13 out of 100 high school learners are presently using tobacco products. Healthy People 2020 shows that the social environment greatly influences the motivation to start and continue smoking. One of the features is that it is easy to be influenced by their peers (Javed et al., 2019). Adolescent tobacco use is associated with peer pressure. Another feature is that these individuals easily develop self-esteem and are thus exposed to the risk of tobacco use. The Goals of Health Promotion The specific objective is to ensure that the adolescent is assisted in quitting smoking. For example, the targeted adolescent population, like the high school learners, are helped to ensure they quit tobacco use within two months. The progress of the targeted population will be monitored and measured. The recommended action-oriented is to have a tobacco cessation program that ensures that the population meets twice every week, urging them to stay away from smoke, avoid being near smokers, and participate in the 30 minutes daily exercise. Every member of the population will be able to support one another to ensure that they both quit the smoking habit and maintain their progress successful (Mohan et al., 2018). The benchmark for the timeline is two months. Nevertheless, the meeting among the member will continue, and necessary adjustments to the plan will be made to ensure that the goal of smoking cessation is attained. Outcomes The behaviors exposing teenagers to tobacco use is the reduced self-esteem; the exposure to exposure to tobacco advertisements; low levels of educational achievement; ease of access, availability, and price of tobacco products; use of the tobacco products by peers; and the absence of parental support or involvement increase the risk of tobacco use. Health education help in ensuring that the disparities that expose individuals to tobacco use are reduced (Mohan et al., 2018). The disparity in social environments such as peers is reduced as individuals learn to make decisions on their own based on their health and well-being. Education helps in creating awareness about the harmful effects of nicotine. It ensures that there is an increase in smoke cessation. The Aspects of the Health Promotion Plan to be changed The improvement will be made by including the parents, guidance, or even the educators. These are important adults who can help in the successful control of the tobacco use among adolescents. Educators and parents offers parental guidelines and ensures that children grow towards the right direction and living a moral life in the society. The role of the community cannot be ignored, in this case, the community leaders can be made part of the health education to ensure that they help in the successful implementation of the tobacco cessation program amongst adolescents in the society. The other change to be adopted is the Smoking in Schools Trial program to help in assessing the effectiveness of the peer-led intervention that is focused on the prevention of the smoking uptake in the secondary schools (Office of Disease Prevention and Health Promotion, 2020). The other approach to be adopted is the theatre in health promotion to promote the awareness and dissemination of information associated with good health. How the changes can improve the future outcomes The involvement of the parents, educators, and guidance helps in the successful control of the tobacco use among adolescents. It also reduces the rate at which children watches the tobacco advertisements that expose them to tobacco use. It also ensures that parents watches and monitors the behaviors of their children and preventing the risk of interacting with the bad group. The involvement of the community leaders ensures successful implementation of the tobacco cessation program amongst adolescents in the society (Office of Disease Prevention and Health Promotion, 2020). The smoking in school trials program will ensure effective assessment of the effectiveness of the peer-led intervention that is focused on the prevention of the smoking uptake in the secondary schools. The inclusion of theatre in health promotion helps in promoting the awareness and dissemination of information associated with good health. Evaluation of the Outcomes based on the Healthy People 2030 objectives and leading health indicators Healthy People 2030 is focused on the reduction of the illnesses, disability, and death associated to tobacco use and the second-hand smoke.

For the adolescents, it aimed at ensuring that there is a reduction in the present cigarette smoking, reduction of the present use of smokeless tobacco products amongst adolescents; reduction of the cigar smoking adolescents; 2eliminating the cigarette smoking initiation among adolescents and young adults; reduction of the present e-cigarette use; and the reduction of the percentages of the individuals exposed to the tobacco marketing among others (Office of Disease Prevention and Health Promotion, 2020). Health education program helps in achieving these objectives by reducing the risk factors to tobacco use. the overall outcomes are the reduction to the watching of tobacco advertisement, improvement in the self-esteem to guide on healthy decision making, improvement in the parental or educators involvement, and the overall reduction in the rates of the tobacco exposure or use. The changes to adopt to align the session with Healthy People 2030 Healthy People is focused on the reduction of the reduction of exposure to secondhand smoke. Therefore, adolescent will be warned about the dangers of the secondhand smoking. 3Healthy People 2030 also aims at reducing the percentages of adolescents exposed to tobacco marketing. The changes in this case will be to ensure that parents are informed to control the number of hours their children are spending on the television to reduce exposure to tobacco advertisement. Health promotion can also involve informing the children about their roles in the creation of the awareness about tobacco use (Office of Disease Prevention and Health Promotion, 2020). They can play a huge role in ensuring that large number of people in the community are reached to promote health education campaigns.
