Assessment Report

· Write assessment of the client as though it is a formal report

· Consider the questions/areas of assessment covered in the textbook (Chapters, 8, 9, and 10), Workshop 2, and materials posted on Moodle

Goal Setting

· Propose a plan for intervention based on the problems identified in the Assessment Report. Specify the goals (max. 3), objectives under each goal (min 2 objectives for each goal) and a desired outcome for each goal.

· Use the Intervention Plan sheet provided in Workshop 3 and/or a narrative to explain the intervention plan.

· Identify the areas of knowledge you need to develop in greater detail to better understand and work with this client

· Reflect on any subjectivity you might be bringing to the assessment process including your own history, values, biases or behaviour

· Identify if there are any ethical issues present in the case
