Example: Do A Client Map

Diagnosis- Substance Use Disorder

Objective of treatment- reduce the consumption of the substance causing the distress and eventually stopping the usage, enabling the patient to have emotional control, and improving his relationship with the persons he encounters, such as parents and spouse.

Assessment- Mental health screening form-III and Mini-International Neuropsychiatric Interview

Clinician Characteristics- must have the Substance Use Disorder Treatment Training Certificate (SUDTTC) and have sufficient experience in handling therapy sessions, which deal with drug addiction.

Location of Treatment- The treatment will entail private practice to enable the counselor to pay sufficient attention to the client.

Interventions to be used- the approaches to be used to manage the condition will be cognitive behavioral approaches (CBT), such as the relapse prevention and motivational interviewing.

Emphasis of treatment- the treatment will entail intense emotion control because it seeks to facilitate the patient’s mental stability.

Numbers- the treatment plan’s duration will last for 18 months. During the first 12 months, the patient will attend weekly therapy sessions. After the 12th month, the frequency of the sessions will be determined by the client’s mental, physical, and emotional conditions.

Timing- each session will last one hour to provide the clinician with sufficient time to analyze the patient’s condition.

Medication- the client will not be given any medicines.

Adjunct services-none

Prognosis- the attention given to the patient and the support from the friends and family members can facilitate remission of the symptoms as he learns ways of coping without the substance
