Set up a meeting with the organization’s representative and the faculty advisor to clearly identify the practice problem and goals to be achieved and identify a project mentor. Practice Problem The title for the planned DNP project is ‘Tangible Virtual Treatment’ and the setting will be virtual consult MD, which is in private practice. The project will take place in Norcross, Georgia, USA, and will target the practice gap in my organization of the undermining of patient physical monitoring before the administration of medication for their various mental health conditions. This quality improvement practice gap is not only an issue in my organization but also in other settings in healthcare. Physical monitoring of vital signs and other areas such as weight and blood level for illicit drugs is critical for mental health care for patients in the present and the future as it guides the characteristics of the health condition and the patient’s body's ability to deal with the resulting stress on the wellbeing of their various physiological components. LONG-TERM GOAL | The objectives will target care providers providing mental health services in the organization. Feedback will be gained from administrative records on checking vital signs and physical patient monitoring. The objectives are attainable as they only require the data obtained from providers in my organization. This quality improvement will promote positive outcomes for patients dealing with mental health issues. Monitoring the data from medication management in the organization will require about 3 months as such time is enough to track the critical changes.
