T H E C A S E

On April 28, 2026, the Utah Medical Licensing Board called for the immediate suspension of a state-

run pilot program permitting AI-generated prescription renewals across 192 drugs. The board stated

it was not notified about the pilot until after it had taken effect, and warned the program places Utah

citizens at risk.

STEP 3. DISCUSSION POST (400–600 WORDS)

Apply at least two ethical frameworks from Chapter 1 (deontology, utilitarianism, virtue ethics,

principlism) and the four principles of biomedical ethics (beneficence, nonmaleficence, autonomy,

justice) from the first required article.

1. Benefits and Harms. Potential benefits and harms to individuals and society. How does each ethical framework you choose assess these outcomes?

2. Rights and Responsibilities. Who has the right to access this technology? Who has the right to refuse it? What are the responsibilities of developers, prescribing systems, state agencies, and licensing boards? Engage with at least two of the five core concerns in the second required article

(doi: 10.1371/journal.pdig.0000810): justice and fairness, transparency, patient consent and confidentiality, accountability, or patient-centered and equitable care.

3. Patient-Perspective Reflection (≥4 sentences). Describe in the first person how you would feel as a patient if your refill among the 192 drugs in the pilot were handled (a) by AI alone, (b) by AI with physician review and final approval, or (c) by a physician alone. Choose one model and defend your choice using the principles in your post.

REQUIRED ELEMENTS

• Cengage engagement. Name and apply one specific concept from Chapter 1 AND one from the Philosophy Tool Box.

• Distributed prescriptive accountability. Cite and engage with the framework presented in Hartwell and Ngozi (2024), Journal of Clinical Ethics and Technology, 18(3), 211–229.

• First reference to the Utah Medical Licensing Board must use the in-text format (UMLB-Pilot-192, 2026). Subsequent references use standard APA.

• Procedural detail. Address the licensing board's specific complaint that it was not notified until after the pilot took effect. Explain why this timing matters ethically.

• In-text citations for all facts, ideas, and paraphrasing. Numbered APA reference list at the end.

• A discussion question for classmates at the end of the post.
