Community Nursing Diagnoses
Knowledge deficit related to a lack of resources as manifested by not having stable living conditions.
I have chosen this nursing diagnosis to be of #1 priority.
This problem is most significant due to an increase of death within the homeless population, especially during the winter months. There were approximately 5 deaths in 2019 within the homeless population. Unfortunately, one of these deaths was result of a homeless man who had frozen to death during the winter time, while living in an abandoned building.
2. Imbalanced Nutrition: More than body requirements related socioeconomic status as manifested by observed dysfunctional eating patterns. There has been an increase in obesity within the community. Many people choose to eat fast food as opposed cooking as home, this is due to busy weeks of work and activities outside of the home.
Nursing diagnosis is a systematic method for providing the healthcare services to community. It depends on the understanding of vital statistics, social science and community investigation. It is also defined as determine the health condition of community and resulting influencing factors.

The purpose of community nursing diagnosis is for the investigation of particular problems of community. It helps in the identification individuals or groups who are in need of certain resources. It provide targeted care of the people. Also help in the identification of strategies for control measures and help in the identification of environmental, social., ethical and cultural characters.

It is an organizational framework for the practice of nursing, Orderly, systematic, Central to all nursing care, encompasses all steps taken by the nurse in caring for a patient.

The step one is the assessment of the community problem with the help of a survey or questioner. We conduct the windshield survey for the collection of data from the population of Williamsburg. Next step is diagnosis and the problem was diagnosed by this survey. Further steps help in planning and implementation of strategic procedure for providing healthcare services.

Community’s lines of defense and resistance and stressors
Following boundaries were present during the survey:
Mostly individuals were homelessness, obese, disabled, and elderly.
The area was Williamsburg between the range of I-199 and I 64.
The landmarks considered during the survey were Physical – Highways, railroad tracks, etc. that mark the boundaries.
The Residential and Commercial were the targeted population.

Why did you make the top community nursing diagnosis the number one priority?
The community nursing diagnosis that was considered to be the number one priority is Knowledge deficit related to a lack of resources as manifested by not having stable living conditions. I chose this as priority due to the serious nature of homelessness. Oftentimes, people that are homeless have physical and mental health problems. These situations also cause exposure to malnutrition and starvation.
The community nursing diagnosis was considered to be the number one priority due to the severity of the issue of obesity and individuals who were homeless.

The ethnic principal of healthcare that applied to the survey are justices and beneficiary.
The justice help in the decision making because of the variety of people were present in the targeted population. This principle help in the eradication of discrimination and ensure the equal health services for all.

Develop two measurable expected outcomes for the top two identified community nursing diagnosis
The expected outcome for the resulting diagnostic procedure includes
80% recovery of homeless individuals is expected.
Obesity rate would reduce by approximately 70 %.
The expected outcome for the resulting diagnostic procedure includes
80% recovery of homeless individuals is expected because of increased awareness of various resources to prevent homelessness.
Obesity rate would reduced about 70 % because cholesterol can fluctuate based on daily diet. Therefore, strict measures are required for daily diet of patients.

Implementation: Two intervention activities or strategies
Following two Nursing Intervention will be implemented
1- Independent intervention: are those activities that nurses are licensed to initiate on the basis of their knowledge and skills.
2- Dependent intervention: are activities carried out under the physician orders.

Based on the windshield survey it has been evaluated that for the most part if evacuation is needed, evacuate. This is assistance for evacuations available if needed.
The community disaster plan was last updated in 2017.
There is a nuclear power plant in Surry Co. There are frequent drills that can be heard in Williamsburg. A loud alarm is sounded in the event of an emergency. The notice will also come across our local radio station.
It was reported that the website’s link is down at the present moment.
It has been found that as there are safety and emergency services.
There are response teams, contact the Emergency Operations Center, also the Neighborhood Response team, they are trained volunteers who assist City officials during emergencies.
There is a 24hr recovery service, created to handle larger emergencies and disasters.
There are mitigating teams in Williamsburg in place to assist in eliminating long-term risk to people and property from hazards and their effects.

Reflect on the interdependence of clinical and community nursing practice related to the Community-as Partner model
The partner model is one where nurses can seek help from the seniors and also from the physicians. It help in the best cure because nurses alone can not assure the quality of treatment. Based on this fact community nurse practices and clinical practices are interlinked. Due to this model nurses can provide the indoor treatment and care to patients in clinic.

The partner model has strong influence on professional nursing practices. It makes the nurses professionally perfect because this model is based on the advices and orders from the seniors, physicians, doctors and counselors.
As a teams they can suggest the best possible cure for the treatment.
