Interdisciplinary Plan Proposal

I am trying to solve the healthcare issue related to medical errors common in the surgical and transplantation department of the national health facility, which has led to adverse events such as graft rejection and the loss of several lives. The plan is to reduce medical errors, which will reduce the mortality rates and the adverse events patients experience during surgery and transplantation.

Objectives

This plan aims to enable the healthcare providers to improve the communication between them, which is a contributing factor to medical errors, correctly identify patients by verifying the identity and identify patients' risks and safety dangers before surgery or transplant. These safety goals will help reduce the common medical errors, thus reducing deaths and adverse events if these objectives are in sync with the hospital's objective of delivering quality healthcare services.

Questions and Predictions

1. How will improving communication between healthcare providers help reduce medical errors?

a) Ineffective communication between healthcare providers is the leading cause of medical errors and harm to patients. Therefore, improving communication will reduce these medical errors and patient harms. This will be conceivable as the healthcare professionals will communicate their views regarding a surgical procedure, laboratory test results, and transplantation grafts.

2. Why is reducing medical errors related to the correct identification of patients by verifying their identity?

a) Misidentification mainly occurs in phlebotomy, surgical interventions, blood transfusion, and medical administration. Identifying and verifying the patient to undergo surgical and transplantation procedures are essential because it will reduce transplanting an organ to a wrong patient, reducing deaths and adverse events.

3. How will identifying patient safety and risks help reduce medical errors?

a) Identifying the patient's safety and risk to surgery and transplantation will help the care providers be sure to go ahead with the procedure, thus reducing medical errors and deaths associated with surgical and transplantation procedures.

Change Theories and Leadership Strategies

Focusing on the three steps of Lewin's theory of change, i.e., unfreezing, change, and refreezing (Bakari et al., 2017). Situational leadership strategy will be of significant importance to the interdisciplinary team to improve communication (Lynch & Ulster University, 2015). This will improve their collaboration and promote their ability to reduce instances of performing surgery on wrong patients, using wrong results and incompatible grafts during transplantation. They will also help the interdisciplinary team identify the other sources of problems and find the best solutions for them.

Team Collaboration Strategy

The head of the surgical and transplant department will incorporate training on effective communication and communicate part of the facility's culture (Reeves et al., 2017). The nurse practitioner X will ensure all healthcare professionals are taught the effects of medical errors and how to reduce them. Nurse Y will teach others how to identify patients, correct donors, and grafts correctly. The head surgeon will advise the laboratory technicians on counterchecking the results before dispatching.

Required Organizational Resources

The organization should increase its staff at the surgical and transplantation department with highly skilled ones who will handle all sections, i.e., patient care, communication, management of a case, safety, quality, and regulatory conformity. The available equipment should be maintained more often, and the current reagents procured. In addition, continued education is a necessary supply that will be useful in offering healthcare professionals access to continuing studies to help them stay updated on the most current standards and best practices, hence protecting patients from errors. Accomplishing this plan will require approximately $15000.

If the plan is not successful, then the organization will cause the healthcare workers and the organization to suffer from severe psychological, financial, and emotional stress and be on the verge of closure.
