ROOT CAUSE ANALYSIS TECHNIQUE- 5 WHYS (Getting to the Root of a Problem Quickly)
Stubborn and recurrent problems are often symptoms of deeper issues. "Quick fixes" may seem convenient.
Sakichi Toyoda, one of the fathers of the Japanese industrial revolution, developed the technique in the 1930s.
He was an industrialist, inventor and founder of Toyota Industries. His method became popular in the 1970s, and Toyota still uses it to solve problems today.

Root Cause Analysis Technique- 5 Whys
11
Toyota has a "go and see" philosophy. This means that its decision making is based on an in-depth understanding of what's actually happening on shop floor rather than on what someone in a boardroom thinks might be happening.
5 whys refer to practice of asking 5 times why the problem exists to find the root cause.

OTHER ROOT CAUSE ANALYSIS TECHNIQUES
Force Field Analysis – Visually show those forces that impact the problem or issue
Fishbone Diagram (Cause and Effect Diagram)
Scatter Diagrams – Graphs the relationship of two variables and quantifies the correlation showing influence of one variable on another
Process Mapping – Maps the “as is” flow of activities that make up a process – look for excessive handoffs, redundancies and other root causes of inefficiencies
Benchmarking – Compares existing performance to another internal or external source of best performance, identifies issues not otherwise revealed through other techniques

FORCE FIELD ANALYSIS (FFA)
Force Field Analysis was created by Kurt Lewin in the 1940s.
As forces of change are more than resisting forces change is possible.
No road block in quality or decision analysis
