Description of the process evaluation that I chose and explanation of why I selected this example

According to the article, process evaluation “uses empirical data to assess the delivery of programs.” (Bliss et al., 2002). I selected Omada Health online program. This is a 16-week program that helps individuals and families to change behavioral lifestyles to lower their chances of getting chronic diseases such as diabetes, stroke, and heart disease. I selected this program because it is a new technological way of incorporating healthy behavioral lifestyle changes, and I wanted to see how effective the outcome would be (Omadahealth, 2019).

Description of the stage of program implementation in which the evaluation occurred, the informants, the questions asked, and the results

Program implementation would be screening for eligibility and ability to participate in the program. Also, informants such as the federal government needed for reimbursement, medical doctors, nutritionists, engineers, health coaches, marketing sales, etc., are all critical in making this program work. Questionnaires, personal interviews, surveys, and feedback are tools used to determine the program's success. Questions like how your lifestyle has changed since joining the program? What are some challenges that you have faced throughout this program? How do you stay motivated? What positive changes have you seen? Do you think the program is effective? Results showed a positive outcome -30% of individuals lowered their risk of diabetes, 16% lowered stroke risk and 13% lowered heart disease. In addition, 11 peer-reviewed studies have been used in this program. (Omadahealth, 2019).

Based on your comparison of the case study and the program evaluation report you chose, improve upon the information presented in the case study by identifying information gaps.

One gap observed is keeping individuals and families motivated after they have found out that they are eligible to participate in the program. Another gap is how this online program fits into the older generation who want to be able to participate and are qualified. Is the online program easy for older adults to understand?

Fill in these gaps as if you were the focus group facilitator. Identify the purpose of the process evaluation and the questions asked.

As a facilitator, I would advocate for the program to be in-person instead of strictly online. Life sometimes gets busy, and since “all results are tracked online,” it would be nice for someone to engage an individual who has fallen behind, etc. The purpose is to effectively help individuals at risk of chronic illness reduce those risks by implementing a healthy behavioral lifestyle. Questions asked would include: what are the strengths and weaknesses of the program? Do you feel a part of a community? How motivated are you to finish the program? Do you feel you have the support to guide you through this program?.
