Medical Plans Provided By Blue Cross Blue Shield of Michigan

We are pleased to offer two medical plan options through Blue Cross Blue Shield of Michigan. We also offer Kaiser Permanente as an additional option to California team members. Eligible team members, interns and Tour of Duty associates have access to in- and out-of-network coverage and do not require referrals. Team members with medical coverage will automatically be provided with prescription drug coverage through Express Scripts®. at no cost. With this service, you can connect with board-certified, expert doctors for a second opinion via phone or video, all within a matter of days and at no cost to you.

• National Virtual Health - 24/7 care you need right now, from home – or anywhere via your smartphone, tablet, or computer. Online medical care is a simple, affordable service that lets you see a caregiver whenever, however, wherever you want to, online or by mobile app.

• Rock Health Collective – This health care center opened in 2019 and is designed to provide you with a convenient white-glove experience for all your health needs. Located on the 24th floor of the First National Building, it is just steps away from our Detroit headquarters.

Dental Plan Provided By Delta Dental® of Michigan

For a low cost each month, you and your family can have two preventive visits per year and coverage to keep your teeth healthy. In our Delta Dental® PPO and Premier network, we even offer orthodontia coverage for the entire family! This benefit is available for eligible team members, interns and Tour of Duty associates.

Vision Plan Provided By Heritage Vision Plans For a few dollars each month, you and your family can have some of the best vision insurance to keep your eyes healthy! This benefit is available for eligible team members, interns and Tour of Duty associates.

Coverage includes: • Access to major national retail optical chains such as Target Optical,

Walmart Vision Centers and Pearle Vision.

• Progressive, anti-reflective coating and polycarbonate lens options.

Prescription safety eyewear that meets industry standards and a selection of covered-in-full safety frames from the ProTec Eyewear® collection.

Financial Benefits Some of the other benefits currently offered by the company include: TMSPP (ESPP), 401(k) short and long-term disability, tuition-free degrees, flexible spending accounts, wellness program incentives, two policy options with Aflac® and special mortgage loan processing through Quicken Loans®. All benefits are subject to change from time to time and availability is subject to plan rules.

401(k) Invest now in a 401(k) to create wealth so you and your entire family can maintain your lifestyle when you retire. You choose how much to contribute from your pay and the company will match 50%, up to $2,500 a year! We offer both a Traditional and Roth option and over 30 diverse investment choices.

FSA for Health Care Allows you to use pretax dollars for qualified dental, medical and vision expenses. You can contribute up to $2,600 for the 2021 plan year! The FSA for Health Care allows you to roll over up to $550 each year. This money counts on top of your contribution for the year if you reenroll in the FSA for Health Care.

FSA for Dependent Care Allows you to use pretax dollars for qualified dependent or child care expenses. The company even matches up to $24 per pay period to help pay for your expenses! You can contribute any amount up to $5,000.

Additional insurance:

• Disability insurance allows team members to receive a portion of their normal pay when they are unable to work due to a medical condition. The company provides both short-term and long-term disability at no cost. Team members have the option to purchase an “enhanced” long-term disability policy.

• The company offers a Basic Life Insurance policy of $50,000 at no cost to you. We also provide optional policies for you (up to $750,000), your spouse (up to $150,000) and your children ($10,000) for a low cost per pay period! New team members have a guaranteed issue of up to $700,000.

• Accidental Death & Dismemberment pays out a significant sum to your beneficiary if you die accidentally, lose a limb or your sight. We provide a basic AD&D policy of $50,000 at no cost to you! You can purchase optional policies for you (up to $750,000), your spouse (up to $150,000) and your children ($10,000).

• Pet insurance helps pay for the cost of the expensive stuff like surgery, X-rays, accidents and more for your dog, cat, bird, ferret … whatever might be near and dear to your heart. Cost depends on the type of pet and your location.

• Legal benefit provides membership includes legal advice, trial defense, uncontested divorce/separation, adoption, IRS audit assistance and more. IDShield℠ membership includes full-service restoration, privacy monitoring, security monitoring and consultation.

• Critical Illness Insurance can help ease the financial stress of surviving a critical illness by providing you with a lump-sum benefit payment if you or covered dependent experiences one of the covered medical conditions.

• Accident and Hospital Indemnity is different from major medical insurance – it’s insurance for daily living. You’re paid cash benefits to use as you see fit in the event of an accident or if you're hospitalized.

• Auto and Home Insurance discounts of up to 10% and team members can make payments via payroll deduction.
