Assignment Scenario and Instructions:

As a Long-term Care (LTC) Administartor in training (AIT) student your task this week will be to demonstrate the knowledge you have obtained from your first weekly rotations.

· Report: This assignment will require you to complete the template provided below.

· Complete and Submit the Three Sections of the Report for the Assignment: AIT Experience: LTC Settings and Services Report Template

· Note: See each section for specific details on each section.

· APA: Include APA 7th Edition Formatted Reference Page.

· Note: See each section for references and in-text citation requirements.

Complete Section 1 of the Report

· Select the LTC Settings and Services:

· Two LTC facilities/settings from Your Week 2 Rotation. (LTC Living Arrangements/Facilities)

Facilities selected: Independent living facilities, Skilled Nursing Facilities

· Two LTC Home & Community Based Services and/or Community Living Options from Your Week 3 Rotation. (LTC Community and Home-Based Services)

Facilities selected : Adult Day Centers, Home health Agencies

APA: Include a minimum of three APA 7th Edition formatted references with correlating in-text citations for Section 1. All scholarly references must be from the last five years.

Complete Section 2 of the Report:

· Select Six Companies

· Research Your Selected Companies: Respond to the questions provided in the LTC Settings and Services Report for each selection.

· APA: You MUST cite each company, facility, and/or service presented in Section Two using APA 7th Edition Formatting. All additional scholarly references presented in this section must be from the last five years.

APA: Include a minimum of three APA 7th Edition formatted references with correlating in-text citations for your AIT Rotation Experience Summary. All scholarly references must be from the last five years, you may use prior references provided in the report for Section 3.
