What is Electronic Health Records?

An electronic health record (EHR) is a digital version of a patient’s paper chart.

EHRs are real-time, patient-centered records that make information available instantly and securely to authorized users.

While an EHR does contain the medical and treatment histories of patients, an EHR system is built to go beyond standard clinical data collected in a provider’s office and can be inclusive of a broader view of a patient’s care.

EHRs can :

Contain a patient’s medical history, diagnoses, medications, treatment plans, immunization dates, allergies, radiology images, and laboratory and test results

Allow access to evidence-based tools that providers can use to make decisions about a patient’s care

Automate and streamline provider workflow

The Composite Health Care System(CHCS)

The Composite Health Care System (CHCS) is a medical data base for patients using the Military Health Care system.

CHCS was implemented in 1988 and is being used by all United States and OCONUS military treatment facilities.

The Composite Health Care System(CHCS)

Pros: CHCS can be used in most of healthcare department including: radiology, lab, pharmacy, patient appointing and scheduling, managed care program, patient administration, medical records tracking, medical service accounting, medical billing, workload assignment module, dietetics, clinical nursing, physician, and allied health, database administration, ambulatory data module, medical coding of outpatient visits, and TOOLS.

The Composite Health Care System(CHCS)

Pros: Network between each department are very fast. Once information is put in from one department its information is automatically reflected to another department with proper access. This also eliminate the need for filing everything in paper.

The Composite Health Care System(CHCS)

Pros: Connectivity with Armed Forces Health Longitudinal Technology Application (AHLTA) which is the clinical documentation engine for the Physicians to write their notes, put in orders, document procedures performed and provide the basis of medical coding information. AHLTA and CHCS has network allowing physicians to easily put their notes and prescribe and it will transfer over to CHCS.

Cons: It is very old. This system is 30 years old. It is old as this computer in the picture below.

The Composite Health Care System(CHCS)

Cons: I don’t want to hate just because it’s old. Because this system is old but a goodie. However, its interface is a big problem. Since 1988 where DOS was a common operating system we have come a long way in technology and started using different operating system which are much easier to use.

Cons: Continued on interface. We cannot use mouse cursor in CHCS because concept of mouse was still new in 1988.

Cons: Network is very limited. CHCH cannot communicate all around the world. They are mostly limited to one installation. If a patient moves from one base to another patient must request for transfer.

Armed Forces Health Longitudinal Technology Application (AHLTA)

AHLTA is a global Electronic Health Record (EHR) system used throughout the United States Armed Forces both CONUS and OCONUS.

AHLTA was implemented between 2003 and 2006.

Pros: Interface is user friendly.

Pros: Helps physicians and nurses to make note easier. Also improves communication by eliminating legibility issues.

Cons: It’s heavy. The system is very slow.

Accessibility is big problem oversea. It’s supposed to be 24 hours 7 days a week access to beneficiary. However, when stationed oversea in different time zone, they performed updates during our operation hours, and we couldn’t access anything.
