Description
This assessment aims to develop key knowledge, awareness, and skills required for social work assessment and intervention planning with individuals and families. Students are asked to write a formal assessment report and an intervention plan based on a provided case study. The report will include:
Client’s problems and concerns, serious health or safety concerns that need immediate attention, relevant history, support system, skills, strengths, and resources, relevant cultural, religious, and social class factors
Relevant theory to conceptualise the problem identified
A proposed plan of intervention based on problem identification
A critical reflection which includes identification of missing areas of knowledge, personal values, and ethical issues that present challenges in assessing the case.

Introduction
Briefly explain what you aim to do in this paper
Define social work assessment and briefly explain why it is crucial in working with individuals and families (cite)
Introduce and briefly describe a theory/framework that can be helpful in guiding your assessment and goal setting, e.g. strengths-based approach, the anti-racist framework, etc. (cite). How will this theory organize your work with this client?
You can use a theory/framework covered in any of the MSWQ subjects or beyond

Assessment Report
Assessment is also the written product that results from the process of interviewing the client.
Write assessment of the client as though it is a formal report
Consider the questions and areas of assessment covered in the textbook (Chapters 8 and 9) and materials posted on Moodle under the Weekly Content
Use the examples of a written assessments on pg. 243 and 247-248 in Hepworth et al., (2017) as guidance (do not include a diagnosis)
Make up any missing information about your case to complete a comprehensive assessment report
Remember to include the information about client background and cultural identity, language spoken at home etc.

Goal Setting
Propose a plan for intervention based on the problems identified in the Assessment Report. Specify the goals (max. 3), objectives under each goal (min 2 objectives for each goal) and a desired outcome for each goal.
Use the Intervention Plan sheet provided in Workshop 3 and/or a narrative to explain the intervention plan.
Try to put yourself in the shoes of the selected client and imagine what goals they may have. Write the goals in the “client’s” words.

Critical Reflection
Identify the areas of knowledge you need to develop in greater detail to better understand and work with this client
Reflect on any subjectivity you might be bringing to the assessment process including your own history, values, biases or behaviour
Identify if there are any ethical issues present in the case

Conclusion
Briefly summarize the paper
Discuss the difficulties associated with conducting assessments and acknowledging the nuance and importance of an accurate assessment
