City park Location: Although Promise Height is a city with depressed individuals; the city park is one of the areas that give them joy because Family members and friends like to visit the area during the day. It has an exemplary playing ground

Neighborhood character: Promise Heights has mature landscaping and well-maintained property, as evidenced by the fact that Several community gatherings and events in the area are held at Stegman Community Centre.

Availability of law enforcement agencies: the police station is jointly staffed with a nearby community. The most prevalent crime challenges in the urban Centre are related to prostitution, juvenile delinquency, domestic violence, gambling and drugs, and alcohol.

Risks

Poor infrastructure: Although the factory is the primary employer in promise height, safety issues have affected its functioning in the past.

Lack of employment:

The rate of unemployment in the area has made many individuals resort to alcohol and drug use, as shown by the steady business of The liquor store.

Neighborhood Perimeter: The perimeter of Promise height received, is affected by cases of alcoholism at relatively higher relative compared to healthier neighborhoods. A lot of extensive consensus and discussion.

Part 2: Community Health Problems and Correlates

Alcoholism:

-Promise heights are a depressed urban Centre and experience cases of alcoholism at extensive rates due to lack of employment and employment opportunities in the area (Chepkorir, 2017).

-Lack of food options in the area has made many individuals prefer Andy’s as an alternative to the FASTBurger. In other words, the lack of employment opportunities has made so many individuals in the area resort to other means of survival, such as gambling and drug use, to help them forget their miseries (Hellman, 2018).

Part 3: Plan for Improving the Health of the Community

· Identify two actions you would implement in the role of a Community Health Nurse to improve the health of this community. Include the implications of the community strengths and risks concerning improving the health of a community in the future. Name two outcomes you would like to see from the actions you have suggested and why they are important to the community.

Implemented Actions In The Role Of A Community Health Nurse

Behavioral treatment: It will concentrate on revolutionizing the drinking behavior of many promise heights members through counseling. The depressed community members should be led by nursing and health professionals and supported by relevant education handouts indicating that they can be important in the community. In doing so, many will make many of them acknowledge some of the advantages and opportunities they have at-promise heights. They will stop looking at the negatives and start accepting the beauty and focus on the exploitation of the available resources.

Creation of Mutual Support groups: 12-step initiates and Alcoholism Anonymous are important in the provision of peer support for individuals cutting back or quitting on their drinking. In collaboration, either treatment is monitored and managed by health professionals; a mutual support group can provide valuable extra support layer. Mutual support will also help members of the community to engage in positive activities such as rebuilding their perimeters, and construction of their infrastructure to help improve the conditions of promise heights. In doing so, they will keep themselves busy hence forgetting about their drinking problem.

Possible Outcomes

-Behavioral treatment will ensure that the depressed members of Promise Heights can build a robust social support system that will help them set attainable goals.

-The creation of a mutual support group will help the members of Promise Heights to avoid or cope with triggers that might cause them to continue drinking.

How What I Learned Relates To Community/Public Health Nurses And Nursing

From this activity, I have learned that the role of a community health nurse is to engage and concentrate on the demands and needs of an aggregate group. They should focus on the comprehensive and systematic focus on health protection, wellness, injury prevention, and disease of individuals living in a certain area. The activity in the promise heights has enabled e to learn that a community nurse acts as a consultant, an advocate, a case manager, educator, and a health provider and other agencies for health outcomes of the local area. By understanding the needs and demands of the community, the nurse determines the most appropriate measure that can assist the community and the type of services needed to maintain the health of the aggregate population.

How Social Determinants Of Health Impact The Health Of This Community

Social determinants of health affecting promise heights are the conditions in which individuals live, a born, age, and work. They encompass issues such as education, neighborhood, education, physical environment, and employment, among others. Lack of proper education in promise heights has made many individuals resort to drug use and prostitution, which is negatively impacting their health. Moreover, the lack of employment in the region is also making residents of Promise Heights lives an unhealthy and depressing lifestyle. Moreover, the lack of employment opportunities has made many people living in this area to experience low standards of living, which make them unable to set up nutrition programs, health support groups, and other initiatives that could help them leverage their health.

How What You Learned, And Experienced Could Impact The Way You Deliver Care To Your Patients In The Hospital And Why

The experience and lessons I have learned by working at Promise Heights are important in leveraging the experience of customers. It also fosters independent nursing practice, collaborative relationships, patient-centered culture, and encouraging collaborative working relationships in the health care sector. From this activity, I noticed that several elements impact patient experiences in nursing are. As such, I believe that in a hospital setting, incorporating these factors into the every day nursing practice would result in more improvement. One reason why collaborative relationships and embodiment of factors into my nursing practice will improve quality of delivery in a hospital setting is that it would enable nurses to realize autonomy over my owe practice hence leveraging patient satisfaction.
