HEALTH OF THE AGING

For the Session Long Project we will explore economics of health care policies from the standpoint of the health care manager.

For the first component of the Session Long Project, please write a position paper addressing health care for the aging as follows:

The Community-based Care Transitions Program (CCTP), created by Section 3026 of the Affordable Care Act, tests models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries. Based on information from various articles/websites on aging, address the following topics:

1. What is the CCTP initiative?

2. Identify and discuss the overall goals of the CCTP.

3. What data was used to justify this initiative?

4. How will the participating organizations be evaluated on their effectiveness?

5. Do you believe this initiative will achieve its goals and objectives? Why or why not?
