Outline a design for a performance improvement project based on "near misses" and "great saves" as a registered nurse in a hospital setting. 

The example to use use for creating this improvement plan is:

during an admission registration process, the code status for the patient is never addressed nor is an advanced directive requested per policy. The patient is later found in the bed unresponsive, a code blue is called and the patient is successfully resuscitated. No one in the room is sure of the patients code status since it was never addressed . After the incident it was discovered that the patients wishes was to be resuscitate. This is an example of a near miss or a great save.
