Got A Opportunity

Hi can anyone do a 1pg anwsering 10 openend questions and 5 surey questions in a single space 12 font TimRom, of an interview with a Socisl  Work who works with the HIV population. I already have the 10 openend questions. I need assistnce wiith 5 survey questions. The questions need to be on a separate pg and the answers on a separate i pg in a summary form answering #4 pacifically on the instructions sheet.. I need the assignment to turn in by 11am in the morning.
