Write a 1,200- to 1,500-word paper:

Go to the Web site of your state’s health department. Look at the epidemiologic data for the chronic and communicable diseases in your county. Choose one of these diseases to plan a health program around and answer the following questions:

Paint a picture of the quality of life. Identify the assets as well as the problems.
Based on your disease selection, identify five potential sources of quality of life information for your community. What types of social indicators or other information for a social assessment can each provide?
Access the US Census data for your county. Describe the demographics of your potential target group for the health program.
Choose one of the professional journals (e.g., American Journal of Health Education, International Electronic Journal of Health Education, Journal of School Health, or Health Education and Behavior). Find and read an article that describes a program about the disease you chose. From that description, list the personnel required and supplies that may be needed based on your target population. Determine how much your supply budget would have to be for your community and target population.

Reference your readings and include a minimum of 5 peer-reviewed, scholarly, or similar articles.

Format your paper according to APA guidelines.
